ABSTRACT

OBJECTIVE@#To investigate the relationship among the gross motor function classification system (GMFCS)and the development of hip joint and lumbar spine in children with spastic cerebral palsy.@*METHODS@#The clinical data of 125 children with spastic cerebral palsy admitted from January 2018 to July 2021 were retrospectively analyzed. There were 85 males and 40 females, aged from 4 to 12 years old with an average of (8.4±2.9) years. According to GMFCS, the patients were divided into gradeⅠ, Ⅱ, Ⅲ and Ⅳ groups. There were 27 cases in gradeⅠgroup, 40 cases in gradeⅡgroup, 35 cases in grade Ⅲ group and 23 cases in grade Ⅳ group. The migration percentage(MP), central edge angle(CE), neck-shaft angle(NSA), acetabular index(AI) were measured by the radiograph of pelvis, abnormal parameters were selected to evaluate the relationship between different GMFCS grades and hip joint development. Lumbar sagittal Cobb angle, lumbar sacral angle, lumbar lordosis index and apical distance were measured by lateral lumbar radiographs to evaluate the relationship between different GMFCS grades and lumbar spine development.@*RESULTS@#①Among the 125 spastic cerebral palsy children, there were 119 cases of pelvic radiographs that met the measurement standards. In the four groups with gradeⅠ, Ⅱ, Ⅲ, Ⅳ, MP was (22.72±3.88), (26.53±4.36), (33.84±4.99), and (49.54±7.87)%, CE was(30.10±6.99) °, ( 22.92±4.19) °, ( 17.91±5.50) °, and (-0.70±17.33)°, AI was (16.41±2.77) °, (20.46±4.63) °, (23.76±5.10) °, and ( 29.15±7.35)°, respectively, there were significant differences between the two comparisons (P<0.05). And the higher GMFCS grade, the greater MP and AI, and the smaller CE.The NSA was(142.74±10.03) °, (148.66±9.09) °, (151.66±10.52) °, and (153.70±8.05)° in four groups with gradeⅠ, Ⅱ, Ⅲ, Ⅳ, respectively. The differences between the two comparisons of the GMFCS gradeⅠgroup and the other three groups were statistically significant (P<0.05). NSA of GMFCSⅠgroup was significantly lower than that of the others, there was no significant difference among other groups(P>0.05). ② Among the 125 spastic cerebral palsy children, there were 88 cases of lumbar spine radiographs that met the measurement standards. ③The lumbar sagittal Cobb angle was(32.62±11.10) °, (29.86±9.90) °, (31.70±11.84) °, and (39.69±6.80)° in the four groups with gradeⅠ, Ⅱ, Ⅲ, Ⅳ, respectively;GMFSS of grade Ⅳ group was significantly higher than that of other three groups, there was significant difference between the two comparisons (P<0.05);there were no significant differences between other groups (P>0.05). In the four groups with gradeⅠ, Ⅱ, Ⅲ, Ⅳ, the lumbosacral angle was (31.02±9.91) °, ( 26.57±9.41) °, (28.08±8.56) °, and ( 27.31±11.50)°, the lumbar lordosis index was (4.14±12.89), (8.83±13.53), (13.00±11.78), and (10.76±9.97) mm, the arch apex distance was (9.50±6.80), (6.68±3.20), (7.16±4.94), and (6.62±4.13) mm, respectively, there were no significant differences between the two comparisons(P>0.05).@*CONCLUSION@#①In children with GMFCS gradeⅠ-Ⅳ, the higher the GMFCS grade, the worse the hip develops. ② Children with GMFCS grade Ⅲ-Ⅳ may be at greater risk for lumbar kyphosis.

ABSTRACT

OBJECTIVE@#To determine whether C7 angles (C7 slope, C7S) could replace T1 angles (T1 slope, T1S) by correlation analysis of T1S and C7S.@*METHODS@#A total of 442 patients from July 2015 to July 2020 in outpatient and inpatient department were enrolled retrospectively, and 259 patients who could identify the upper endplate of T1 were screened out . Of them, there were 145 males and 114 females, aged from 20 to 83 years old with an average of (58.6±11.2) years, including 163 patients with cervical spine surgery and 96 non-surgical patients. Patients were stratified by sex, age, cervical kyphosis, cervical alignment imbalance, and cervical spine surgery. These 259 patients included 145 cases in the male group, 114 cases in the female group;76 cases in the youth group (<40 years old), 109 cases in the middle-aged group (40 to 60 years old), and 74 cases in the elderly group(>60 years old); 92 cases in the cervical kyphosis group, 167 cases in the non-kyphosis group;51 cases in the cervical sequence imbalance group, 208 cases in the non-imbalance group;163 cases in the cervical surgery group, 96 cases in the non-operation group. The correlations of C7S and T1S in various modalities groups were analyzed.@*RESULTS@#Of 442 patients, the recognition rate of upper endplate of T1 was 58.6%(259/442), and that of C7 was 90.7%. The mean T1S and C7S of the 259 patients were (24.5±8.0)° [(25.9±7.7)° in the male group and (23.7±6.9)° in the female group] and (20.8±7.3)° [(22.5±7.5)° in the male group and(19.7±5.8)° in the female group], respectively. The total correlation coefficient between C7S and T1S was r=0.89, R2=0.79, and the linear regression equation was T1S=0.91×C7S+4.35. In the above general information and the grouping of deformity factors, T1S was highly correlated with C7S(r value 0.85 to 0.92, P<0.05).@*CONCLUSION@#There is a high correlation between T1S and C7S in different factor groups. For cases where T1S cannot be measured, C7S can be used to provide guidance and reference for evaluating the sagittal balance of the spine, analyzing the condition, and formulating surgical plans.

OBJECTIVE@#To compare the short-term clinical efficacy and radiologic differences between oblique lateral interbody fusion(OLIF) and minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) for degenerative lumbar spondylolisthesis.@*METHODS@#A retrospective analysis was performed on 58 patients with lumbar spondylolisthesis treated with OLIF or MIS-TLIF from April 2019 to October 2020. Among them, 28 patients were treated with OLIF (OLIF group), including 15 males and 13 females aged 47 to 84 years old with an average age of (63.00±9.38) years. The other 30 patients were treated with MIS-TLIF(MIS-TLIF group), including 17 males and 13 females aged 43 to 78 years old with an average age of (61.13±11.10) years. General conditions, including operation time, intraoperative blood loss, postoperative drainage, complications, lying in bed, and hospitalization time were recorded in both groups. Radiological characteristics, including intervertebral disc height (DH), intervertebral foramen height (FH), and lumbar lordosis angle (LLA), were compared between two groups. The visual analogue scale (VAS) and Oswestry disability index (ODI) were used to evaluate the clinical effect.@*RESULTS@#The operation time, intraoperative blood loss, postoperative drainage, lying in bed, and hospitalization time in OLIF group were significantly less than those in the MIS-TLIF group (P<0.05). The intervertebral disc height and intervertebral foramen height were significantly improved in both groups after the operation (P<0.05). The lumbar lordosis angle in OLIF group was significantly improved compared to before the operation(P<0.05), but there was no significant difference in the MIS-TLIF group before and after operation(P>0.05). Postoperative intervertebral disc height, intervertebral foramen height, and lumbar lordosis were better in the OLIF group than in the MIS-TLIF group (P<0.05). The VAS and ODI of the OLIF group were lower than those of the MIS-TLIF group within 1 week and 1 month after the operation (P<0.05), and there were no significant differences in VAS and ODI at 3 and 6 months after the operation between the two groups(P>0.05). In the OLIF group, 1 case had paresthesia of the left lower extremity with flexion-hip weakness and 1 case had a collapse of the endplate after the operation;in the MIS-TLIF group, 2 cases had radiation pain of lower extremities after decompression.@*CONCLUSION@#Compared with MIS-TLIF, OLIF results in less operative trauma, faster recovery, and better imaging performance after lumbar spine surgery.

OBJECTIVE@#To investigate the changes in spinal-pelvic sagittal parameters from preoperative standing to prone position in old traumatic spinal fractures with kyphosis.@*METHODS@#The clinical data of 36 patients admitted between December 2016 and June 2021 for surgical treatment of old traumatic spinal fractures with kyphosis, including 7 males and 29 females, aged from 50 to 79 years (mean, 63.9 years), were retrospectively analyzed. Lesion segments included 2 cases of T 11, 12 cases of T 12, 2 cases of T 11, 12, 4 cases of T 12 and L 1, 12 cases of L 1, 2 cases of L 2, 1 case of L 2, 3, and 1 case of L 3. The disease duration ranged from 4 to 120 months, with an average of 19.6 months. Surgical procedures included Smith-Petersen osteotomy in 4 cases, Ponte osteotomy in 6 cases, pedicle subtraction osteotomy in 2 cases, and improved fourth level osteotomy in 18 cases; the remaining 6 cases were not osteotomized. The bone mineral density ranged from -3.0 to 0.5 T, with a mean of -1.62 T. The spinal-pelvic sagittal parameters from preoperative standing to prone positions were measured, including local kyphosis Cobb angle (LKCA), thoracic kyphosis (TK), lumbar lordosis (LL), sacral slope (SS), pelvic tilt (PT), and PI and LL mismatch (PI-LL). The kyphotic flexibility=(preoperative standing LKCA-preoperative prone LKCA)/preoperative standing LKCA×100%. Spinal-pelvic sagittal parameters were compared between standing position and prone position before operation, and Pearson correlation was used to judge the correlation between the parameters of standing position and prone position before operation.@*RESULTS@#When the position changed from standing to prone, LKCA and TK decreased significantly ( P<0.05), while SS, LL, PT, and PI-LL had no significant difference ( P>0.05). Pearson correlation analysis showed that LL was significantly correlated with SS and PI-LL in both standing and prone positions ( P<0.05), and the correlation strength between LL and SS in prone position was higher than that in standing position. In the standing position, LKCA was significantly correlated with SS and PT ( P<0.05). However, when the position changed from standing to prone, the correlation between LKCA and SS and PT disappeared, while PT and PI-LL was positive correlation ( P<0.05). The kyphotic flexibility was 25.13%-78.79%, with an average of 33.85%.@*CONCLUSION@#For the patients of old traumatic spinal fractures with kyphosis, the preoperative LKCA and TK decrease significantly from standing position to prone position, and the correlation between spinal and pelvic parameters also changed, which should be taken into account in the formulation of preoperative surgical plan.

OBJECTIVE@#To investigate the feasibility of predicting proximal junctional kyphosis (PJK) in adults after spinal deformity surgery based on back-forward Bending CT localization images and related predictive indicators.@*METHODS@#A retrospective analysis was performed for 31 adult patients with spinal deformity who underwent posterior osteotomy and long-segment fusion fixation between March 2017 and March 2020. There were 5 males and 26 females with an average age of 62.5 years (range, 30-77 years). The upper instrumented vertebrae (UIV) located at T 5 in 1 case, T 6 in 1 case, T 9 in 13 cases, T 10 in 12 cases, and T 11 in 4 cases. The lowest instrumented vertebrae (LIV) located at L 1 in 3 cases, L 2 in 3 cases, L 3 in 10 cases, L 4 in 7 cases, L 5 in 5 cases, and S 1 in 3 cases. Based on the full-length lateral X-ray film of the spine in the standing position before and after operation and back-forward Bending CT localization images before operation, the sagittal sequence of the spine was obtained, and the relevant indexes were measured, including thoracic kyphosis (TK), lumbar lordosis (LL), local kyphosis Cobb angle (LKCA) [the difference between the different positions before operation (recovery value) was calculated], kyphosis flexibility, hyperextension sagittal vertical axis (hSVA), T 2-L 5 hyperextension C 7-vertebral sagittal offset (hC 7-VSO), and pre- and post-operative proximal junctional angle (PJA). At last follow-up, the patients were divided into PJK and non-PJK groups based on PJA to determine whether they had PJK. The gender, age, body mass index (BMI), number of fusion segments, number of cases with coronal plane deformity, bone mineral density (T value), UIV position, LIV position, operation time, intraoperative blood loss, osteotomy grading, and related imaging indicators were compared between the two groups. The hC 7-VSO of the vertebral body with significant differences between groups was taken, and the receiver operating characteristic curve (ROC) was used to evaluate its accuracy in predicting the occurrence of PJK.@*RESULTS@#All 31 patients were followed up 13-52 months, with an average of 30.0 months. The patient's PJA was 1.4°-29.0° at last follow-up, with an average of 10.4°; PJK occurred in 8 cases (25.8%). There was no significant difference in gender, age, BMI, number of fusion segments, number of cases with coronal plane deformity, bone mineral density (T value), UIV position, LIV position, operation time, intraoperative blood loss, and osteotomy grading between the two groups ( P>0.05). Imaging measurements showed that the LL recovery value and T 8-L 3 vertebral hC 7-VSO in the PJK group were significantly higher than those in the non-PJK group ( P>0.05). There was no significant difference in hyperextension TK, hyperextension LL, hyperextension LKCA, TK recovery value, LL recovery value, kyphosis flexibility, hSVA, and T 2-T 7, L 4, L 5 vertebral hC 7-VSO ( P>0.05). T 8-L 3 vertebral hC 7-VSO was analyzed for ROC curve, and combined with the area under curve and the comprehensive evaluation of sensitivity and specificity, the best predictive index was hC 7-L 2, the cut-off value was 2.54 cm, the sensitivity was 100%, and the specificity was 60.9%.@*CONCLUSION@#Preoperative back-forward Bending CT localization image can be used to predict the occurrence of PJK after posterior osteotomy and long-segment fusion fixation in adult spinal deformity. If the patient's T 8-L 2 vertebral hC 7-VSO is too large, it indicates a higher risk of postoperative PJK. The best predictive index is hC 7-L 2, and the cut-off value is 2.54 cm.

ABSTRACT

OBJECTIVE@#To introduce a scout view scanning technique of back-forward bending CT (BFB-CT) in simulated surgical position for evaluating the remaining real angle and flexibility of thoracolumbar kyphosis secondary to old osteoporotic vertebral compression fracture.@*METHODS@#A total of 28 patients with thoracolumbar kyphosis secondary to old osteoporotic vertebral compression fracture who met the selection criteria between June 2018 and December 2021 were included in the study. There were 6 males and 22 females with an average age of 69.5 years (range, 56-92 years). The injured vertebra were located at T 10-L 2, including 11 cases of single thoracic fracture, 11 cases of single lumbar fracture, and 6 cases of multiple thoracolumbar fractures. The disease duration ranged from 3 weeks to 36 months, with a median of 5 months. All patients received examinations of BFB-CT and standing lateral full-spine X-ray (SLFSX). The thoracic kyphosis (TK), thoracolumbar kyphosis (TLK), local kyphosis of injured vertebra (LKIV), lumbar lordosis (LL), and the sagittal vertical axis (SVA) were measured. Referring to the calculation method of scoliosis flexibility, the kyphosis flexibility of thoracic, thoracolumbar, and injured vertebra were calculated respectively. The sagittal parameters measured by the two methods were compared, and the correlation of the parameters measured by the two methods was analyzed by Pearson correlation.@*RESULTS@#Except LL ( P>0.05), TK, TLK, LKIV, and SVA measured by BFB-CT were significantly lower than those measured by SLFSX ( P<0.05). The flexibilities of thoracic, thoracolumbar, and injured vertebra were 34.1%±18.8%, 36.2%±13.8%, and 39.3%±18.6%, respectively. Correlation analysis showed that the sagittal parameters measured by the two methods were positively correlated ( P<0.001), and the correlation coefficients of TK, TLK, LKIV, and SVA were 0.900, 0.730, 0.700, and 0.680, respectively.@*CONCLUSION@#Thoracolumbar kyphosis secondary to old osteoporotic vertebral compression fracture shows an excellent flexibility and BFB-CT in simulated surgical position can obtain the remaining real angle which need to be corrected surgically.

La hipocondroplasia es una displasia esquelética caracterizada por baja estatura, constitución robusta, brazos y piernas desproporcionadamente cortos, manos y pies anchos y cortos, leve laxitud articular y macrocefalia. Los niños generalmente se presentan como pequeños, con velocidad de crecimiento disminuida, que conduce a una baja estatura y desproporción de las extremidades. La hipocondroplasia en la mayoría de los casos se hereda con carácter autosómico dominante, aunque se detectan numerosos casos esporádicos. El diagnóstico requiere una exhaustiva anamnesis y adecuada exploración física. Es importante valorar algunos indicadores de crecimiento como: peso para la edad, longitud/talla para la edad, relación entre peso y longitud/talla, velocidad de crecimiento, talla diana genética, medidas de segmentos corporales, entre otros. Las radiografías esqueléticas permiten diagnosticar la mayoría de las displasias óseas. Los estudios moleculares suelen ser la prueba de confirmación y se solicitan ante una sospecha diagnóstica. Es importante incluir las displasias óseas en el diagnóstico diferencial de la talla baja y tenerlas en cuenta ante cualquier caso de talla baja disarmónica con alteraciones fenotípicas. La hipocondroplasia en la actualidad, no es una indicación aprobada para tratamiento con hormona del crecimiento. Se presenta un caso clínico de una niña de 14 meses, con talla baja severa, desproporcionada, que presentó dificultades para llegar al diagnóstico definitivo de hipocondroplasia.


Hypochondroplasia is a skeletal dysplasia characterized by short height, robust build, disproportionately short arms and legs, short and broad hands and feet, mild joint laxity, and macrocephaly. Children generally show slow growth rate, which leads to short stature and limb disproportion. Hypochondroplasia is mostly inherited with an autosomal dominant character, although many sporadic cases have been detected. Diagnosis requires a thorough history and adequate physical examination. It is important to assess some growth indicators such as: weight for age, length/height for age, relationship between weight and length/height, growth speed, genetic target height, measurements of body segments, among others. Skeletal XRs can diagnose most bone dysplasias. Molecular studies are usually the confirmatory test and are requested when a diagnosis is suspected. It is important to include bone dysplasias in the differential diagnosis of short stature and to take them into account for any disharmonious short stature with phenotypic alterations. Hypochondroplasia is currently not an approved indication for growth hormone therapy. We present a clinical case of a 14-month-old girl, with a severe, disproportionate short stature, who presented difficulties in her definitive hypochondroplasia diagnosis.

As crianças geralmente são pequenas, com diminuição da velocidade de crescimento, o que leva à baixa estatura e desproporção dos membros. A hipocondroplasia na maioria dos casos é herdada com caráter autossômico dominante, embora sejam detectados numerosos casos esporádicos. O diagnóstico requer uma história completa e um exame físico adequado. É importante avaliar alguns indicadores de crescimento como: peso para idade, comprimento/altura para idade, relação entre peso e comprimento/altura, taxa de crescimento, estatura alvo genético, medidas de segmentos corporais, entre outros. As radiografias esqueléticas permitem o diagnóstico da maioria das displasias ósseas. Os estudos moleculares são geralmente o teste de confirmação e são solicitados quando há suspeita de diagnóstico. É importante incluir as displasias ósseas no diagnóstico diferencial da baixa estatura e considerá-las em qualquer caso de baixa estatura desarmônica com alterações fenotípicas. A hipocondroplasia não é atualmente uma indicação aprovada para o tratamento com hormônio de crescimento. Apresenta-se o caso clínico de uma menina de 14 meses, com baixa estatura grave e desproporcional, que apresentou dificuldades em chegar ao diagnóstico definitivo de hipocondroplasia.

Abstract Objectives To verify if there is a difference in postural hypervigilance in sitting in individuals with and without low back pain. Additionally, to observe whether there is a difference in the perception of correct sitting posture between individuals with low back pain and without low back pain. Methods The present study has a cross-sectional observational design, as a sample size of 92 individuals, later divided equally into two groups (with low back pain and without low back pain). Two instruments were used: the hypervigilance scale to analyze the frequency that volunteers correct their sitting posture during the day, and posture scans to investigate the perception of volunteers about the correct sitting posture. The data were submitted to the Shapiro-Wilk Normality test. To compare the values of Hypervigilance Scale, the Mann-Whitney, Chi-Square, and Fisher Exact tests were used to assess correct sitting posture. Results There was no significant difference between postural hypervigilance in sitting between individuals with low back pain and without low back pain. There was no significant difference between the choice of correct sitting posture between the group of individuals with and without low back pain. Conclusion There is no difference between the choice of correct sitting posture and the amount of postural hypervigilance in individuals with or without low back pain.


Resumo Objetivos Verificar se há diferença na hipervigilância postural sentada em indivíduos com e sem dor lombar. Além disso, observar se há diferença na percepção da postura correta sentada entre indivíduos com dor lombar e sem dor lombar. Métodos O presente estudo possui delineamento observacional transversal, como tamanho amostral de 92 indivíduos, posteriormente divididos igualmente em dois grupos (com dor lombar e sem dor lombar). Foram utilizados dois instrumentos: a escala de hipervigilância para analisar a frequência que voluntários corrigem a postura sentada no dia; e o quadro de posturas para investigar a percepção dos voluntários sobre a postura correta sentada. Os dados foram submetidos ao teste de Normalidade de Shapiro-Wilk. Para comparar os valores da Escala de Hipervigilância foi utilizado o teste de Mann-Whitney e o teste Qui-quadrado e exato de Fisher para avaliação da postura correta sentada. Resultados Não houve diferença significativa entre a hipervigilância postural sentada entre indivíduos com dor lombar e sem dor lombar. Não houve diferença significativa entre a escolha da postura correta sentada entre o grupo de indivíduos com e sem dor lombar. Conclusão Não há diferença entre a escolha da postura correta sentada e quantidade de hipervigilância postural em indivíduos com ou sem dor lombar.

Abstract Objectives The present study aimed to assess whether preoperative spinopelvic parameters can influence the gain of segmental lordosis after one level of lateral lumbar interbody fusion. Methods The following radiological parameters were measured in the X-rays: pelvic incidence, lumbar lordosis, pelvic tilt, L4S1 lordosis, index level segmental lordosis, intraoperative index segmental lordosis, pelvic mismatch (IP-LL), distal lordosis proportion, delta segmental lordosis, Pelvic Titlt (PT) > 20, actual sacral slope, and ideal sacral slope, and the correlation of these variables with the gain of segmental lordosis was investigated. Afterwards, an exploratory cluster analysis was performed to identify common characteristics between patients and segmental lordosis gain. Results The sample of the present study comprised 104 patients, of which 76% presented segmental lordosis gain. The most correlated parameters with the segmental lordosis gain were preoperative segmental lordosis (−0.50) and delta intraoperative lordosis (0.51). Moreover, patients in the high PI groups had a trend to gain more segmental lordosis (p< 0.05) and a reduced risk of losing segmental lordosis (Odds 6.08). Conclusion Patients with low-medium PI profiles presented higher odds of loss of segmental lordosis. However, the preoperative spinopelvic parameters alone do not seem to play a significant role in the fate of segmental lordosis gain.


Resumo Objetivos O presente estudo teve como objetivo avaliar se os parâmetros espinopélvicos pré-operatórios podem influenciar o ganho da lordose segmental após fusão intersomática lombar por via lateral de um nível. Métodos Os seguintes parâmetros radiológicos foram medidos nos raios X: incidência pélvica, lordose lombar, versão pélvica, lordose L4S1, lordose segmental do nível operado, índice intraoperatório de lordose segmentar, mismatch pélvico (IP-LL), proporção de lordose distal, delta de lordose segmentar, PT > 20, inclinação sacral real e inclinação sacral ideal, e a correlação dessas variáveis com o ganho da lordose segmentar foi investigada. Posteriormente, foi realizada uma análise exploratória de cluster para identificar características comuns entre os pacientes e o ganho de lordose segmentar. Resultados O presente estudo contou com 144 pacientes, dos quais 76% apresentaram ganho de lordose segmentar. Os parâmetros mais correlacionados com o ganho de lordose segmentar foram lordose segmentar pré-operatória (−0,50) e delta intraoperatório de lordose (0,51). Além disso, os pacientes dos grupos de incidência pélvica (IP) alto tiveram tendência de ganho de lordose segmental maior (p< 0,05) e redução do risco de perda de lordose segmental (chances 6.08). Conclusão Pacientes com perfis de IP médios baixos apresentaram maiores chances de perda de lordose segmentar. No entanto, os parâmetros espinopélvicos pré-operatórios por si só não parecem desempenhar um papel significativo no destino do ganho da lordose segmentar.

Abstract Objective The present study evaluates radiographic outcomes and the lumbar lordosis achieved with a transforaminal lumbar interbody fusion (TLIF) arthrodesis technique according to the positioning of an interbody device (cage) in the disc space. Methods This is a retrospective radiographic analysis of single-level surgical patients with degenerative lumbar disease submitted to a TLIF procedure and posterior pedicle instrumentation. We divided patients into two groups according to cage positioning. For the TLIF-A group, the cages were anterior to the disc space; for the TLIF-P group, cages were posterior to the disc space. Considering the superior vertebral plateau of the lower vertebra included in the instrumentation, cages occupying a surface equal to the anterior 50% of the midline were placed in the TLIF-A group, and those in a posterior position were placed in the TLIF-P group. We assessed pre- and postoperative orthostatic lateral radiographs to obtain the following measures: lumbar lordosis (LL) (angle L1-S1), segmental lordosis (LS) (L4-S1), and segmental lordosis of the cage (SLC). Results The present study included 100 patients from 2011 to 2018; 44 were males, and 46 were females. Their mean age was 50.5 years old (range, 27 to 76 years old). In total, 43 cages were "anterior" (TLIF-A) and 57 were "posterior" (TLIF-P). After surgery, the mean findings for the TLIF-A group were the following: LL, 50.7°, SL 34.9°, and SLC 21.6°; in comparison, the findings for the TLIF-P group were the following: LL, 42.3° (p< 0.01), SL 30.7° (p< 0.05), and SLC 18.8° (p> 0.05). Conclusion Cage positioning anterior to the disc space improved lumbar and segmental lordosis on radiographs compared with a posterior placement.


Resumo Objetivo Avaliar os resultados radiográficos e comparar a lordose pós-operatória em técnica de artrodese intersomática lombar transforaminal (TLIF, na sigla em inglês), considerando como variável o posicionamento do dispositivo intersomático (cage) em relação ao espaço discal. Métodos Análise retrospectiva radiográfica de pacientes cirúrgicos, em nível único, por doença lombar degenerativa, aplicando-se TLIF e instrumentação pedicular posterior. Os pacientes foram divididos, conforme a posição do cage, em 2 grupos: 1. TLIF-A - cages na posição anterior do espaço discal; e 2. TLIF-P, cages na posição posterior do espaço discal (considerando-se o platô vertebral superior da vértebra inferior incluída na instrumentação, cages que ocuparam a superfície correspondente a 50% anterior da linha média, compuseram o grupo TLIF-A; opostamente, cages em posicionamento posterior compuseram o grupo TLIF-P). Procedeu-se à avaliação dos exames radiográficos ortostáticos em perfil no pré- e pós-operatórios, com a tomada das seguintes medidas: lordose lombar (LL) (ângulo L1-S1); lordose segmentar (LS) (L4-S1) e lordose segmentar do cage (LSC). Resultados Cem pacientes foram incluídos de 2011 a 2018, sendo 44 homens e 46 mulheres, com idade média de 50.5 anos (27-76 anos). Um total de 43 cages foram classificados como "anteriores" (TLIF-A) e 57, "posteriores" (TLIF-P); considerando o grupo TLIF- A, os resultados pós-operatórios médios foram: LL 50.7°, LS 34.9° e LSC 21.6°; para o grupo TLIF-P, comparativamente: LL 42.3° (p< 0,01), LS 30.7° (p< 0,05) e LSC 18.8° (p> 0,05). Conclusão O posicionamento anterior do cage em relação ao espaço discal correlaciona-se a melhora da lordose lombar e segmentar na radiografia em comparação com o posicionamento posterior do implante.

Abstract Objectives The present study aims to characterize the spinal balance (SB) in young adults with Schmorl nodes (SN). Methods A cross-sectional study was conducted on a sample of 47 young adults. Lumbar magnetic resonance imaging (MRI) was used to divide the patients into an SN group and a control group. Standing full spine radiographs were used to compare the spinopelvic SB parameters between groups: sagittal vertical axis, thoracic kyphosis, lumbar lordosis (LL), pelvic incidence (PI), pelvic tilt (PT), and sacral slope (SS). Results The LL and SS values were significantly lower in patients with SN when compared with the control group (54.5° versus 64.3°; 36.2° versus 41.4°, respectively). No significant differences were observed for the other parameters. Significant correlations were found in both groups between LL and SS; PI and PT; and PI and SS. Conclusions Young adults with SN have associated SB modifications, particularly lower LL and SS values, when compared with a control group. This flatter profile resembles that observed in patients with lower back pain and early disc pathology. We believe that SNs are relevant clinical findings that should prompt the study of the SB of a patient, as it may uncover variations associated with early disc degeneration. Level of Evidence III


Resumo Objetivos O presente estudo tem como objetivo caracterizar o equilíbrio sagital (SB, na sigla em inglês) espinhal em adultos jovens com nódulos de Schmorl (NS). Métodos Este é um estudo transversal de uma amostra composta por 47 adultos jovens. Ressonância magnética (RM) lombar foi usada para separar os pacientes em um grupo com NS e um grupo controle. Radiografias da coluna vertebral em pé foram usadas para comparar os parâmetros espinopélvicos do SB entre os grupos: eixo vertical sagital, cifose torácica, lordose lombar (LL), incidência pélvica (PI, na sigla em inglês), inclinação pélvica (PT, na sigla em inglês) e inclinação sacral (SS, na sigla em inglês). Resultados Os valores de LL e SS foram significativamente menores nos pacientes com NS em comparação com o grupo controle (54,5° versus 64,3°; 36,2° versus 41,4°, respectivamente). Não foram observadas diferenças significativas nos demais parâmetros. Os dois grupos apresentaram correlações significativas entre LL e SS, PI e PT e PI e SS. Conclusões Adultos jovens com NS apresentam modificações associadas ao SB, principalmente valores menores de LL e SS, em comparação com o grupo controle. Este perfil mais plano assemelha-se ao observado em pacientes com lombalgia e patologia discal em estágio inicial. Acreditamos que o NS seja um achado clínico relevante que deve levar ao estudo do SB de um paciente por poder revelar variações associadas aos primeiros estágios de degeneração discal. Abstract Objective The present study aims to determine the intra- and inter-rater reliability and reproducibility of the Roussouly classification for lumbar lordosis types. Methods A database of 104 panoramic, lateral radiographs of the spine of male individuals aged between 18 and 40 years old was used. Six examiners with different expertise levels measured spinopelvic angles and classified lordosis types according to the Roussouly classification using the Surgimap software (Nemaris Inc., New York, NY, USA). After a 1-month interval, the measurements were repeated, and the intra- and inter-rater agreement were calculated using the Fleiss Kappa test. Results The study revealed positive evidence regarding the reproducibility of the Roussouly classification, with reasonable to virtually perfect (0.307-0.827) intra-rater agreement, and moderate (0.43) to reasonable (0.369) inter-rater agreement according to the Fleiss kappa test. The most experienced examiners showed greater inter-rater agreement, ranging from substantial (0.619) to moderate (0.439). Conclusion The Roussouly classification demonstrated good reliability and reproducibility, with intra- and inter-rater agreements at least reasonable, and reaching substantial to virtually perfect levels in some situations. Evaluators with highest expertise levels showed greater intra and inter-rater agreement.


Resumo Objetivo Determinar a confiabilidade e reprodutibilidade intra- e interavaliadores da classificação dos tipos de lordose lombar de Roussouly. Métodos Foram utilizadas 104 radiografias panorâmicas da coluna vertebral em incidência de perfil, de banco de dados, de indivíduos do sexo masculino com idade entre 18 e 40 anos. Utilizando o software Surgimap (Nemaris Inc., Nova York, NY, EUA), seis examinadores com diferentes níveis de experiência aferiram os ângulos espinopélvicos e classificaram o tipo de lordose de acordo com a classificação de Roussouly. Após um intervalo de 1 mês, as mensurações foram realizadas novamente, sendo calculadas as concordâncias intra- e interavaliadores através do teste Kappa de Fleiss. Resultados O estudo demonstrou evidências positivas em relação à reprodutibilidade da classificação de Roussouly, com teste Kappa de Fleiss para concordância intraavaliador entre razoável à quase perfeita (0,307-0,827), e interavaliador entre moderada (0,43) e razoável (0,369). Os examinadores mais experientes apresentaram maior concordância interavaliador, variando entre substancial (0,619) e moderada (0,439). Conclusão A classificação de Roussouly, demonstrou boa confiabilidade e reprodutibilidade, tendo em vista que a concordância intra- e interavaliadores foi considerada no mínimo razoável, podendo atingir os níveis de substanciais à quase perfeitos em algumas situações. Os avaliadores com maior nível de experiência apresentaram maior concordância intra- e interavaliadores.

Objetivo: Comunicar los resultados obtenidos según la posición del dispositivo de TLIF anterior. Materiales y Métodos: Estudio multicéntrico, observacional, analítico, transversal, de recuperación retrospectiva. Se evaluaron los parámetros espinopélvicos pre- y posoperatorios de espinogramas de 20 pacientes que fueron operados entre septiembre de 2019 y agosto de 2021. Se incluyó a pacientes sometidos a artrodesis lumbar con implante de tipo TLIF anterior. Se excluyó a pacientes sin espinograma pre- o posquirúrgico y más de un dispositivo. Resultados: La media de la lordosis monosegmentaria fue de 13,33° antes de la cirugía y de 18,81° después (p <0,001). La media de la lordosis monosegmentaria fue de 7,32°; 2,95° y 6,24° para las posiciones I, II y III, respectivamente. La media de la altura discal fue de 6,22 mm en el preoperatorio y 11,06 mm en el posoperatorio (p >0,001). Conclusiones: Los resultados de la colocación de este tipo de dispositivos y su relación con la lordosis segmentaria fueron alentadores, se comprendió la importancia de la disposición de estos en el extremo anterior del espacio discal. Objective: To report the results obtained according to the position of an anterior TLIF device. Materials and Methods: Multicenter, observational, analytical, cross-sectional, retrospective recovery study. We evaluated the pre and post-operative spinopelvic pa-rameters of the spinograms of 20 patients who underwent surgery between September 2019 and August 2021. Patients who had undergone lumbar arthrodesis with an anterior TLIF implant were included, whereas patients without a pre or post-surgical spinogram and more than one device were excluded. Results: The mean monosegmental lordosis was 13.33° preoperatively and 18.81° postoperatively (p <0.001). The mean monosegmental lordosis was 7.32°, 2.95°, and 6.24° for positions I, II, and III, respectively. The mean disc height was 6.22 mm for the preoperative period and 11.06 mm for the postoperative period (p >0.001). Conclusion: We found encouraging results on the placement of this type of device and its relationship with segmental lordosis, understanding the importance of its placement at the anterior end of the disc space. OBJECTIVE@#To observe clinical efficacy of chiropractic manipulation in the treatment of degenerative scoliosis (DS).@*METHODS@#From June 2017 to September 2019, 120 patients with degenerative scoliosis were randomly divided into treatment group (60 cases) and control group(60 cases). The patients in treatment group were treated with chiropractic manipulation once every other day for 4 weeks. The patients in control group were treated with eperisone hydrochloride tablets combined with thoracolumbar orthopedic(TSLO)brace, oral eperisone hydrochloride tablets 50 mg three times a day, wearing TSLO brace for not less than 8 hours a day. The course of treatment was 4 weeks. After the patients were selected into the group, visual analogue scale (VAS) and Oswestry Disability Index (ODI) were recorded before treatment, 1, 2, 3, 4 weeks after treatment and 1 month after treatment. The full length X-ray of the spine was taken before and 4 weeks after treatment, and the scoliosis Cobb angle, sagittal vertical axis (SVA) and lumbar lordosis (LL) were measured and compared. The adverse reactions during the treatment were recorded.@*RESULTS@#There were significant differences in VAS and ODI between two groups at each time point after treatment (P<0.001), VAS and ODI at 2 weeks after treatment (PVAS=0.025, PODI=0.032) and 3 weeks after treatment(PVAS=0.040, PODI=0.044) in treatment group were significantly different from those in control group, but there was no significant difference in VAS and ODI at other time points between treatment group and control group (P>0.05). There was significant difference in Cobb angle between treatment group(P=0.010) and control group(P=0.017) after treatment, but there was no significant difference in LL and SVA between treatment group and control group. There was no significant difference in Cobb angle, LL and SVA between two groups before and after treatment. During the treatment, there were 4 mild adverse reactions in the control group and no adverse reactions in the treatment group.@*CONCLUSION@#Chiropractic manipulation can effectively relieve pain and improve lumbar function in patients with degenerative scoliosis. The onset of action is faster than that oral eperisone hydrochloride tablets combined with TSLO brace, and it has better safety and can improve Cobb angle of patients with degenerative scoliosis.

ABSTRACT Introduction/Objective: The craniovertebral junction (CVJ) requires a detailed evaluation, as the changes in alignment caused by surgery can affect adjacent structures in a secondary way. Examples of these effects are dyspnea or dysphagia after posterior occipitocervical arthrodesis, due to decreased caliber of the oropharynx. These changes can be identified perioperatively by several radiographic parameters that aim to predict possible postoperative respiratory complications. Such complications appear to be related to the narrowest oropharyngeal airway space (nPAS), and may also occur following atlantoaxial (C1-C2) arthrodesis. This work aims to correlate the variation in CVJ alignment parameters before and after C1-C2 arthrodesis with the variation in nPAS. Methods: Patients who underwent posterior C1-C2 arthrodesis between 2011 and 2019 at the National Institute of Traumatology and Orthopedics (INTO) were included in the study, totaling 26 patients. The parameters evaluated included cervical lordosis, C1-C2 angle, slope of C2, Occipito-C2 angle (O-C2), pharyngeal inlet angle (PIA), pharyngeal tilt angle (PTA), occiput and external acoustic meatus to axis angle (O-EAa), cranial transverse motion against C2 angle (C2TA), axial tilt (AT) and the percentage of change in nPAS (%∆nPAS). Results: A correlation was observed between the change in C1-C2 angle, O-C2, PTA, C2TA and the %∆nPAS. Conclusion: The change in cervical alignment and CVJ parameters is correlated with %∆nPAS and should, therefore, be evaluated before and after atlantoaxial fusion as a means of predicting a possible respiratory complication. RESUMO Introdução/Objetivo: A junção craniovertebral (JCV) deve ter avaliação detalhada já que as alterações de alinhamento ocasionadas pela cirurgia podem acometer estruturas adjacentes de forma secundária. Exemplos desses efeitos são dispneia ou disfagia depois de artrodese occipitocervical posterior, por diminuição no calibre da orofaringe. Essas alterações podem ser identificadas no perioperatório por diversos parâmetros radiográficos que visam predizer possíveis complicações respiratórias pós-operatórias. Tais complicações parecem estar relacionadas com o menor espaço da via orofaríngea (nPAS, narrowest oropharyngeal airway space) e também podem ocorrer depois de artrodese atlantoaxial (C1-C2). Este trabalho tem como objetivo correlacionar a variação dos parâmetros de alinhamento da JCV no pré e pós-operatório de artrodese C1-C2 com a variação do nPAS. Métodos: Foram incluídos no estudo pacientes submetidos à artrodese posterior C1-C2 entre 2011 e 2019 no Instituto Nacional de Traumatologia e Ortopedia (INTO), totalizando 26 indivíduos. Os parâmetros avaliados incluíram lordose cervical, ângulo C1-C2, inclinação de C2, ângulo Occipito-C2 (O-C2), ângulo de entrada da faringe (PIA, pharyngeal inlet angle), ângulo de inclinação da faringe (PTA, pharyngeal tilt angle), ângulo do eixo occipital e meato acústico externo (O-EAa, occiput and external acoustic meatus to axis angle), movimento transversal craniano contra o ângulo C2 (C2TA, cranial transverse motion against C2 angle), inclinação axial (AT, axial tilt) e porcentagem de mudança no nPAS (%∆nPAS) resultado: Foi observada correlação entre a mudança dos ângulos C1-C2, OC2, PTA, C2TA e a %∆nPAS. RESUMEN Introducción/Objetivo: La unión craneocervical debe ser objeto de una evaluación detallada, ya que los cambios de alineación provocados por la cirugía pueden afectar de forma secundaria a las estructuras adyacentes. Ejemplos de estos efectos son la disnea o la disfagia después de la artrodesis occipitocervical posterior debido a la disminución del calibre de la orofaringe. Estos cambios pueden identificarse en el período perioperatorio por varios parámetros radiográficos que pretenden predecir posibles complicaciones respiratorias postoperatorias. Estas complicaciones parecen estar relacionadas con el espacio orofaríngeo más estrecho (nPAS, narrowest oropharyngeal airway space) y también pueden producirse tras la artrodesis atlantoaxial (C1-C2). Este trabajo tiene como objetivo correlacionar la variación de los parámetros de alineación de la unión craneocervical en el período pre y postoperatorio de la artrodesis C1-C2 con la variación del nPAS. Métodos: Se incluyeron en el estudio los pacientes sometidos a artrodesis posterior C1-C2 entre 2011 y 2019 en el Instituto Nacional de Traumatología y Ortopedia (INTO), totalizando 26 individuos. Abstract Objective To measure and document the clinical impact of the waiting time for surgical treatment of patients with spinal deformities in a quaternary center in Brazil. Methods In total, 59 patients with spinal deformity waiting for surgery on our hospital's list were evaluated to observe the impact of the waiting time on the progression of the deformity. Patient evaluation was performed using the SRS-22r questionnaire for health-related quality of life (HRQL) and radiographic images to evaluate the deformity of the spine at the time the patients were included in the waiting list and at the most recent appointment. The radiographic parameters selected for comparison were: Cobb angle of the primary and secondary curves, coronal alignment, apical vertebral translation, pelvic obliquity, sagittal vertebral axis, kyphosis (T5-T12), and lordosis (L1-S1). Results Low HRQL scores according to the SRS-22r questionnaire were observed in patients waiting for surgery. The radiographic parameters showed progression of the deformity on the initial evaluation when compared with the most recent follow-up evaluation. Conclusion The patients waiting for surgical treatment of spinal deformities in our center showed relatively low HRQL scores and radiographic progression of the deformity.


Resumo Objetivo Medir e documentar o impacto clínico do tempo de espera para tratamento cirúrgico de pacientes com deformidades na coluna vertebral em um centro quaternário no Brasil. Métodos No total, 59 pacientes com deformidade espinhal à espera de cirurgia na lista do nosso hospital foram avaliados para observar o impacto dos tempos de espera na progressão da deformidade. A avaliação do paciente foi realizada utilizando o questionário SRS-22r para qualidade de vida relacionada à saúde (QLRS), e imagens radiográficas para avaliar a deformidade da coluna vertebral quando os pacientes foram incluídos na lista de espera e na consulta mais recente. Os parâmetros radiográficos selecionados para comparação foram: ângulo de Cobb de curvas primárias e secundárias, alinhamento coronal, translação de vértebra apical, obliquidade pélvica, eixo vertebral sagital, cifose (T5-T12), e lordose (L1-S1). Resultados Baixos escores de QLRS segundo o questionário SRS-22r foram observados em pacientes que aguardavam cirurgia. Os parâmetros radiográficos mostraram progressão da deformidade na avaliação inicial em comparação com a avaliação de seguimento mais recente. Conclusão Os pacientes que aguardavam tratamento cirúrgico de deformidade espinhal em nosso centro apresentaram os escores de QLRS relativamente baixos e progressão radiográfica da deformidade.

BACKGROUND@#Finding an optimal treatment strategy for adolescent idiopathic scoliosis (AIS) patients remains challenging because of its intrinsic complexity. For mild to moderate scoliosis patients with lower skeletal growth potential (Risser 3-5), most clinicians agree with observation treatment; however, the curve progression that occurs during puberty, the adolescent period, and even in adulthood, remains a challenging issue for clinicians. The aim of the study is to investigate the efficacy of Schroth exercise in AIS patients with lower skeletal growth potential (Risser 3-5) and moderate scoliosis (Cobb angle 20°-40°).@*METHODS@#From 2015 to 2017, data of 64 patients diagnosed with AIS in Peking University Third Hospital were reviewed. Forty-three patients underwent Schroth exercise were classified as Schroth group, and 21 patients underwent observation were classified as observation group. Outcomes were measured by health-related quality of life (HRQOL) and radiographic parameters. HRQOL was assessed using the visual analog scale (VAS) scores for back, Scoliosis Research Society-22 (SRS-22) patient questionnaire. Radiographic spinopelvic parameters were obtained from anteroposterior and lateral X-rays. The pre-treatment and post-treatment HRQOL and radiographic parameters were tested to validate Schroth exercise efficacy. The inter-rater reliability of the radiographic parameters was tested using the interclass correlation coefficient (ICC). The paired t test was used to examine HRQOL and radiographic parameters. Clinical relevance between C2-C7 sagittal vertical axis (SVA) and thoracic kyphosis was analyzed using Spearman correlation.@*RESULTS@#In Schroth group, VAS back score, SRS-22 pain, and SRS-22 self-image domain were significantly improved from pre-treatment 3.0 ± 0.8, 3.6 ± 0.5, and 3.5 ± 0.7 to post-treatment 1.6 ± 0.6 (t = 5.578, P = 0.013), 4.0 ± 0.3 (t = -3.918, P = 0.001), and 3.7 ± 0.4 (t = -6.468, P < 0.001), respectively. No significant improvements of SRS-22 function domain (t = -2.825, P = 0.088) and mental health domain (t = -3.174, P = 0.061) were observed. The mean Cobb angle decreased from 28.9 ± 5.5° to 26.3 ± 5.2° at the final follow-up, despite no statistical significance was observed (t = 1.853, P = 0.102). The mean C2-C7 SVA value decreased from 21.7 ± 8.4 mm to 17.0 ± 8.0 mm (t = -1.224 P = 0.049) and mean T1 tilt decreased from 4.9 ± 4.2 ° to 3.5 ± 3.1° (t = 2.913, P = 0.011). No significant improvement of radiographic parameters and HRQOL were observed in observation group.@*CONCLUSIONS@#For AIS patients with a Risser 3-5 and a Cobb angle 20°-40°, Schroth exercises improved HRQOL and halted curve progression during the follow-up period. Both cervical spine alignment and shoulder balance were also significantly improved after Schroth exercises. We recommend Schroth exercises for patients with AIS.

ABSTRACT Objective To present a new principle for correction of the sagittal plane of the spine through the convergent or divergent placement of monoaxial pedicle screws in this plane, associated with compression or distraction, to provide lordotizing or kyphotizing leverage force. Method A statistical mechanical study of twenty-eight fixations in synthetic spine segments was performed. In fifteen pieces, pedicle screws were applied to the ends of the segments with positioning convergent to the center of the fixation. They were attached to the straight rods and subjected to compression force. The other thirteen segments were fixed with pedicle screws in a direction divergent to the center of the fixation, attached to the straight rods, and subjected to distraction force. Results To create kyphosis in the 15 synthetic segments of the spine, the mean pre-fixation Cobb angle was - 0.7° and the mean post-fixation angle was +15°. To create lordosis in the 13 segments, the mean pre-fixation Cobb angle was +1° and the mean post-fixation angle was +18°. The difference was confirmed by statistical mechanical tests and considered significant. However, there is no relevant difference between the mean angles for lordosis and kyphosis formation. Conclusions It was concluded that the correction of the sagittal plane of the spine by applying the new instrumentation method is efficient. A statistical mechanical test confirmed that the difference in Cobb degrees between pre- and post-fixation of the synthetic spine segments was considered significant in the creation of both kyphosis and lordosis. RESUMO Objetivo Apresentar um novo princípio para correção do plano sagital da coluna vertebral, posicionando os parafusos pediculares monoaxiais nesse plano de forma convergente ou divergente, associados à compressão ou distração, para proporcionar força em alavanca lordotizante ou cifotizante. Métodos Realizou-se um estudo de mecânica estatística de 28 fixações em segmentos de coluna sintética. Em quinze peças, foram aplicados parafusos pediculares monoaxiais nos extremos dos segmentos abordados com posicionamento no sentido convergente à fixação. Foram agregados às hastes retas e submetidos à força de compressão. Em outros treze segmentos, a fixação foi feita com parafusos pediculares monoaxiais, no sentido divergente ao centro da fixação, integrados às hastes retas e submetidos à força de distração. Resultados Para criar cifose nos 15 segmentos sintéticos da coluna vertebral, a média do ângulo de Cobb na pré-fixação foi de -0,7° e a média pós-fixação foi de +15°. Para cria lordose em 13 segmentos, a média do ângulo de Cobb na pré-fixação foi de +1° e a média pós-fixação foi de +18°. A diferença foi confirmada por testes de mecânica estatística e considerada significativa. Contudo, não existe diferença relevante entre os ângulos médio para formação da lordose e da cifose. Conclusões Conclui-se que a correção do plano sagital da coluna aplicando o novo método de instrumentação é eficiente. Confirmou-se com teste de mecânica estatística que a diferença em graus de Cobb entre o período pré e o pós-fixação dos segmentos de coluna sintética fixados foi considerada significativa, tanto na criação da cifose quanto da lordose. Métodos Se realizó un estudio de mecánica estadística de 28 fijaciones en segmentos de columna sintética. En quince piezas, fueron aplicados tornillos pediculares monoaxiales en los extremos de los segmentos abordados con posicionamiento en el sentido convergente a la fijación. Fueron agregados a las varillas rectas y sometidos a la fuerza de compresión. En otros trece segmentos, la fijación fue hecha con tornillos pediculares monoaxiales, en el sentido divergente del centro de la fijación, integrados a las varillas rectas y sometidos a la fuerza de distracción. Resultados Para crear cifosis en los 15 segmentos sintéticos de la columna vertebral, el promedio del ángulo de Cobb en la prefijación fue de -0,7° y el promedio de postfijación fue de +15°. Para crear lordosis en 13 segmentos, el promedio del ángulo de Cobb en la prefijación fue de +1° y el promedio de postfijación fue de +18°. La diferencia fue confirmada mediante tests de mecánica estadística y considerada significativa. Sin embargo, no existe diferencia relevante entre los ángulos promedios para la formación de lordosis y de cifosis. Conclusiones Se concluye que la corrección del plano sagital de la columna aplicando el nuevo método de instrumentación es eficiente. Se confirmó con test de mecánica estadística que la diferencia en los grados de Cobb entre el período de pre y postfijación de los segmentos de columna sintética fijados fue considerada significativa, tanto en la creación de cifosis como de la lordosis. ABSTRACT Objective The objective of this study is to analyze the radiographs of patients who underwent anterior lumbar interbody fusion (ALIF), to compare the values of the lumbopelvic measurements, and to quantify improvements in these parameters achieved through this technique. Methods The radiographs of 42 patients, all submitted to ALIF with a 12° interbody device, were evaluated from a database at a single center. The pelvic incidence, pelvic tilt, sacral slope, lumbar lordosis, segmental lordosis, and regional lordosis angles of each patient were measured in pre- and postoperative radiographs. Results We observed a discreet change in the pelvic parameters and a marked increase in regional lordosis with a mean increase of 5.8° (p>0.001). Segmental lordosis also showed a mean increase of 2.43°. The gain in segmental lordosis was even higher in patients with degenerative spondylolisthesis and when the operated level was L5-S1. Conclusions The ALIF technique in the lumbar spine is capable of significantly increase the lordosis of a segment, whether at one or two levels. Greater improvement in the lumbopelvic parameters was observed it the procedures performed in level L5-S1 and in cases that presented spondylolisthesis.


RESUMO Objetivo O presente estudo tem por objetivo analisar as radiografias de pacientes submetidos à artrodese intersomática via anterior (ALIF), comparar os valores das medidas lombopélvicas e quantificar a melhora nesses parâmetros oferecida através dessa técnica. Métodos Foram avaliadas as radiografias de 42 pacientes de um banco de dados de um único centro, todos submetidos à ALIF com dispositivo intersomático de 12º. Foram aferidos os valores dos ângulos de incidência pélvica, inclinação pélvica, inclinação sacral, lordose lombar, lordose segmentar e lordose regional na radiografia pré- e pós-operatória de cada paciente. Resultados Observou-se uma alteração discreta nos parâmetros pélvicos e um aumento expressivo na lordose regional com um aumento médio de 5,8º (p>0,001). A lordose segmentar também demonstrou aumento com uma média de acréscimo de 2,43º. O ganho da lordose segmentar foi ainda maior nos pacientes com espondilolistese degenerativa e quando o nível operado era L5-S1. Conclusões A técnica de ALIF na coluna lombar é capaz de aumentar significativamente a lordose de um segmento, seja em um ou dois níveis. Uma melhora mais expressiva nos parâmetros lombopélvicos foi observada nos procedimentos realizados no nível L5-S1 e em casos que apresentavam espondilolistese.


RESUMEN Objetivo El presente estudio tiene por objetivo analizar las radiografías de pacientes sometidos a la artrodesis intersomática vía anterior (ALIF), comparar los valores de las medidas lumbopélvicas y cuantificar la mejora en esos parámetros ofrecida a través de esa técnica. Métodos Fueron evaluadas las radiografías de 42 pacientes de un banco de datos de un único centro, todos sometidos a la ALIF con dispositivo intersomático de 12º. Fueron medidos los valores de los ángulos de incidencia pélvica, inclinación pélvica, inclinación sacral, lordosis lumbar, lordosis segmentaria y lordosis regional en la radiografía pre y post operatoria de cada paciente. Resultados Se observó una alteración discreta en los parámetros pélvicos y un aumento expresivo en la lordosis regional con un aumento promedio de 5,8º (p>0,001). La lordosis segmentaria también demostró aumento con un promedio de incremento de 2,43º. El aumento de la lordosis segmentaria fue aún mayor en los pacientes con espondilolistesis degenerativa y cuando el nivel operado era L5-S1. Conclusiones La técnica de ALIF en la columna lumbar es capaz de aumentar significativamente la lordosis de un segmento, ya sea en uno o dos niveles. Fue observada una mejora más expresiva en los parámetros lumbopélvicos en los procedimientos realizados en el nivel L5-S1 y en casos que presentaban espondilolistesis.
